According to a recent DRPB report, IT professionals, citing multiple issues, suggested both software failure/network issues and human error as the biggest causes of data loss:

1) Software failure + network failure (50%)
2) Human error (43.5%)
3) Power failure (24%)
4) Weather (14%)

It’s not just about hardware failure!

Also one in four companies never test their DR plans and when they do, more than 65% fail. This makes for sobering reading.
